Lumen ID is located in Seattle, Washington. We are required to charge all Washington
State residents sales tax at the current rate of 8.9%.
Sales tax will only be charged
for orders shipped to a Washington State address. It is computed automatically
in the Shopping
Cart and will be added to your order.
Purchasing items for resale in Washington State:
If you are a Washington State
contractor, designer, electrician, or other entity that plans on reselling our
products, please fill out this
form and return it to us via fax.
Note that you must provide a valid tax ID number. When you place
your order, in the 'Comments' section of the payment process write "Resale
Certificate on File". We will then remove any sales
tax charged to your order after it has been placed on the website. The sales
tax will be removed before your credit card is charged for your order.
